Have done a range of customisations from checkout customisations (specify delivery date per item on multi address shipping) through to search customisations (search specific categories directly from header, bit like Amazon) to heavily customised stores (one selling ecards with a CSV upload system for uploading email addresses and a full integration with Mail Chimp for actually sending those ecards out.
