Side menu

A problem that has been bugging me for ages now but why is it so difficult to add a vertical menu on the home page??

For example the http://www.hmv.com with the “browse by department” menu on the left hand side.

Is there a way to do this without having to pay for an extensions?

I tried the vertical navigation extensions but it looked horrible…
