Upgrade from 1.4.1.1 –> 1.6 problems…

Hi everyone,
I’ve got an issue with upgrading to v1.6 that’s very puzzling and really frustrating, any help would be hugely appreciated.
Here’s where we are so far….
About 6 weeks ago we created a test site (v1.6) based on the live one (v1.4.1.1) on our dedicated server. The whole process took around
2 hours and was relatively painless.
After running tests for a few weeks we decided everything was ok and to proceed with the live site.
After about 6 attempts at upgrading the live website, it’s still not done and we’ve had to roll it back each time. Each time the database upgrade is still running after 20 hours.
To try and fix the issue, we run the database repair tool and got the following error:
Change storage engine type on table “catalog_product_index_price_tmp” from INNODB to MEMORY
Add missing foreign key(s) “FK_REPORT_VIEWED_PRODUCT_INDEX_STORE” to table “report_viewed_product_index”
We tried the info of this page to after the error above but it didn’t work either:

And a reindex give the following error:
reindexall gives the following: Column not found: 1054 Unknown column ‘website_date’ in ‘field list’
which relates to catalog_product_index_website
We tried upgrading manually without the upgrade tool, that didn’t work.
We just can’t understand why creating an updating the test website went fine and the live is a nightmare?
Can anyone shed any light on what’s happening, its so frustrating.
